Help information

The form works best on a computer or tablet. You do not need to print it out. Save the form somewhere you can easily find it later.

You can ask a friend, relative, carer, representative, or local support organisation to help you fill in the form over the phone. They should only help you in person if they live with you or visit you for a medical reason or to provide care.
